WHAT YOU WILL NEED TO BRING: Adult SIMPLIFIED RENEWAL (If you have an expired Canadian Passport issued under your current name, in the last six years that has not been damaged, has never been reported lost or stolen, was issued when you were at least 16 years of age, and was valid for 5 years).
- Two identical passport photographs. You are not required to have the photo signed by your guarantor, however, the name and address of the photographer or studio as well as the date the photo was taken must be provided on the back of one photo. A photographer will be onsite to take your photos for a fee of $15. Please do not wear earings or white clothing as these photos will be refused by Passport Canada.

- Payment: For a 24 page passport $87 for adults For a 48 page passport $92 for adults. Passport Canada will accept a credit card (VISA, MasterCard, American Express), money order (postal or bank) or certified cheque made out to the Receiver General of Canada. NO CASH OR DEBIT CARD.
Note:
- If you are applying more than 12 months prior to your passport expiring please provide a written explanation as to why.
- If you would like the submitted passport returned to you, you must indicate this in section 2 of the form.
- Your form and photos must be taken, completed & signed within the past 12 months.
- Please sign within the borders of the signature box. Do not touch the outer limits.
- Your references (in Section 5) cannot be a family member and must have known you at least 2 years.
Adult General Passport Application
- Proof of Citizenship: an original Canadian birth certificate or proof of Canadian citizenship.
- Secondary ID (e.g. health card, driver’s license): You may submit original documents (they will be returned to you). If you are submitting photocopies (both sides), your guarantor must sign & date the copies to indicate that the originals have been seen.
- Photographs: Two identical passport photographs. Your guarantor must write `I certify this to be the true likeness of (your name) `on the back of one of your photos, and sign. The name and address of the photographer or studio as well as the date the photo was taken must be provided on the back of one photo. A photographer will be onsite to take your photos for a fee of $15. Please do not wear earings or white clothing as these photos will be refused by Passport Canada.
- Guarantor: Any person who is 18 years of age or older with a valid passport (or has been expired no more than 1 year) who has known you for at least 2 years. They must have been at least 16 when they applied for their passport. Your guarantor can be a family member.

Child General Passport Application
- Proof of Citizenship: an original Canadian birth certificate or proof of Canadian citizenship. To avoid possible delays, we strongly recommend you provide a long-form birth certificate showing information on both parents (this is required if the child was born outside wed lock).
- All documents that refer to custody or mobility of, or access to, the child. Both parents are requested to participate in obtaining passport services for their child and to sign the application form. Where a court order or agreement exists referring to custody of the child, only the person with custodial rights may apply. If a divorce has been granted, a copy of the divorce judgement or order must also be provided.
- Photographs: Two identical passport photographs. Your guarantor must write `I certify this to be the true likeness of (your name) `on the back of one of your photos, and sign. The name and address of the photographer or studio as well as the date the photo was taken must be provided on the back of one photo. A photographer will be onsite to take your photos for a fee of $15. Please do not wear earings or white clothing as these photos will be refused by Passport Canada.
- Guarantor: Any person who is 18 years of age or older with a valid passport (or has been expired no more than 1 year) who has known you for at least 2 years. They must have been at least 16 when they applied for their passport. Your guarantor can be a family member however if you are the parent or legal guardian applying for the passport for the child you cannot sign as guarantor (however the other parent may).
- Payment: For a 24 page passport $37 for children (ages 3-15), $22 for children (under the age of 3). For a 48 page passport $39 for children (ages 3-15), $24 for children (under the age of 3). Passport Canada will accept a credit card (VISA, MasterCard, American Express), money order (postal or bank) or certified cheque made out to the Receiver General of Canada. NO CASH OR DEBIT CARD.
Note:
- Your form completed & signed within the past 12 months.
- For children under 12 months: To ensure the photo is a current and true likeness of the child, it must be taken within one month of the date the application is submitted.
- Children 11 years or older must sign. Please sign within the borders of the signature box. Do not touch the outer limits.
- The guarantor`s signature of your application (section 2) and one of your photos must be dated the same day or after the dates of your signatures.
- For children under 3 years their passport will be valid for 3 years. For children ages 3 to 15 their passport will be valid for 5 years. Children who have been issued a passport within their first 12 months may be issued a one-time replacement passport, free of charge, valid for the balance of three-year validity period of the original passport.
